Home health nursing is one of the most documentation-heavy specialties in the profession. A registered nurse completing six to eight skilled nursing visits per day can spend between one and three hours after those visits completing OASIS assessments and nursing visit records in WellSky Kinnser or similar EMR platforms. That is time spent staring at a screen after a full day of patient care — instead of resting, preparing for tomorrow, or spending time with family.
The OASIS-E2 (Outcome and Assessment Information Set) is a standardized CMS assessment required for all Medicare and Medicaid home health patients. A single OASIS Start of Care contains more than 21 sections — Patient Tracking, Administrative, Vitals, Patient History and Prognosis, Hearing/Speech/Vision, Cognitive and Behavioral status, Preferences for Customary Routine, Environmental Conditions, Functional Status, Functional Abilities, Bladder and Bowel, Active Diagnosis, Health Conditions, Respiratory, Endocrine, Cardiac, Swallowing and Nutritional Status, Skin Conditions, Medications, Special Treatments, and Supplies. Every section must be completed and saved before the document can be submitted for clinician signature.
WellSky Home Health (formerly Kinnser) is the most widely used home health EMR in the United States. It is thorough and well-integrated with billing, but it is not fast. Each OASIS section is navigated individually, with dropdown menus, multi-field screens, and mandatory fields requiring specific clinical codes. Experienced nurses still spend significantly more time in the platform than they would prefer — especially per-diem and travel nurses completing visits for multiple agencies simultaneously.
After completing a visit, the nurse sends a brief update — typically via WhatsApp — with vitals, any procedures performed, medication changes, and wound photos if applicable. Everything else — patient demographics, referral information, prior assessment data, diagnosis codes, and the bulk of the OASIS sections — is pulled directly from the WellSky Kinnser system. The completed documentation is saved within 24 hours as Pending Clinician Signature. The nurse or supervising clinician reviews and signs. Done.
This service is ideal for per-diem and travel nurses completing high daily visit volumes, home health agencies with documentation backlogs or new staff learning WellSky Kinnser, and LVNs and LPNs completing supervisory visits. The service does not replace clinical judgment — it replaces the clerical and data entry component after the visit is complete.
