Naomi Adebayo — Preeclampsia — Focused Exam

Naomi Adebayo is a pregnant woman presenting with preeclampsia. This focused exam tests your ability to assess and manage a high-risk obstetric emergency — identifying preeclampsia signs, documenting severe features, initiating appropriate interventions and preparing an accurate SBAR handoff to Preceptor Diana.

The subjective assessment covers headache severity and location, visual disturbances (blurred vision, scotomata), epigastric or right upper quadrant pain, oedema progression, fetal movement and understanding of her condition. Objective findings document blood pressure readings (≥140/90 threshold), deep tendon reflexes (hyperreflexia assessment), urinary protein on dipstick, peripheral oedema and fetal heart rate.

Severe features screening is a critical scored domain — documenting whether BP ≥160/110, thrombocytopenia, renal insufficiency, liver enzyme elevation, pulmonary oedema or severe headache unresponsive to medication are present. Magnesium sulfate seizure prophylaxis protocol is initiated and documented.

SBAR handoff covers: Situation (preeclampsia diagnosis), Background (obstetric history, risk factors), Assessment (BP readings, severe features present/absent, fetal status), Recommendation (magnesium sulfate, delivery planning, continuous monitoring).
