Chadwick Bowman is an adult male with COPD and a 40-pack-year smoking history presenting with worsening dyspnoea, productive cough with purulent sputum, fever and oxygen saturation of 91% on room air. The Shadow Health Respiratory Assessment covers a focused pulmonary interview, complete chest examination with auscultation findings including bilateral crackles and decreased breath sounds, COPD exacerbation severity assessment and differential diagnosis covering COPD exacerbation, community-acquired pneumonia and acute bronchitis. The inpatient management plan includes bronchodilator therapy, systemic corticosteroids, supplemental oxygen titration and antibiotic selection with documentation of the clinical reasoning.
